Watch the Challenger Space Shuttle case study video (additional Files or https://www.youtube.com/watch?v=mG8BPB_oPlg&feature=youtu.be ) and then respond to all of topics and questions listed below. 800-1000 words (be thorough but succinct Your response must be created in MS Word format. Use appropriate APA style/structure for this assignment including the use of headings, sections, references, and proper formatting for a paper. You do not need an abstract, but the intro should be a synopsis of the case study and what you will discuss. Please use at least two references overall for this assignment to support your responses. Questions: 1. Considering the Challenger Space Shuttle tragedy that occurred in 1986 (see additional files or https://www.youtube.com/watch?v=mG8BPB_oPlg&feature=youtu.be ), give a synopsis of what you perceive to be the major risks (5 minimum) that should have been considered BEFORE the tragedy occurred. The most obvious would be an “explosion,” so please consider additional risks for your list. You may need to do additional research on this specific incident and/or other projects of this type to develop your list. 2. Using your judgment and based on additional research, how would you rank these risks? Additionally give an indication of probability (High, Medium, Low) of occurring and the impact for each (High, Medium, Low) as this may also help you develop your rankings. Please include the “explosion” risk mentioned in #1 when ranking the risks in addition to those on your list. You may want to consider a table or matrix to show your response. RPI Number: use the “Risk Probability Impact” number (or RPI number) as a way to give a value to your risks – known as “qualitative risk analysis” (yes, ‘qualitative’ and not ‘quantative’ even though you give it a value). Using the RPI number for questions #2 is not required, but is mentioned to provide you with one option to help you rank your risks for this question. If you do decide to calculate an RPI number, you would give the probability a value (such as a % or scale like 1 to 5) and multiple it by the impact, which has also been given a value (such 1 to 5 scale). As an example, a risk that has a high probability of occurring (80% chance) and is determined to have a very high impact (value of 5 on a 1 to 5 scale) would have an RPI number of 0.80 x 5 = 4. In contrast, a risk with a low probability of occurring (10%) and a low impact (1 on a scale of 1 to 5) would have an RPI number of 0.10 x 1 = 0.1. Now, you can make a comparison and rank these risk and focus on the one with the highest RPI number. 3. Were there any risks that should have been taken more seriously and prioritized – – in other words, was there any mounting evidence or other compelling reasons to believe that certain risks had a higher probability of occurring? 4. Were there any “triggers” that indicated a potential risk was becoming a problem or issue (realized risk)? 5. What do you consider to be the primary lessons learned from this horrible tragedy? 6. In your opinion, could this tragedy have been prevented if a thorough risk assessment had been completed and implemented including careful monitoring of triggers and consideration of mounting concerns before the liftoff? Please explain your response.
